“It’s never too late and never too early to plan for daily life after high school.”

Transition is a lifelong process with families and teams to ensure that youth leave school with:

⇒ Communication and language
⇒ Community engagement
⇒ Self-advocacy skills
⇒ IEP transition goals
⇒ Navigation of opportunities and supports for life after HS
⇒ Vocational planning
⇒ Post-Secondary education planning
⇒ Connecting the student, family and team with transition agencies

Sarah Godwin, Transition Consultant, will coordinate with youth and families around next steps to prepare for high school and beyond.
